In an Incident, outcome type could be categorized into three according to the significant risk or substantial harm to the patient / Healthcare workers / other others (visitors,vendors etc)


First outcome type is ;

1 Nearmiss


 According to OSHA, a near miss as an incident in which no property was damaged and no personal injury was sustained, but where, given a slight shift in time or position, damage or injury easily could have occurred.


2 Adverse Incidents 


An adverse incident is an event that causes, or has the potential to cause, unexpected or unwanted effects involving the safety of device users (including patients) or other persons. For example: a patient, user, carer or professional is injured as a result of a medical device failure or its misuse. 


3 Sentinel Incidents 


As per JCI, any unanticipated event in a healthcare setting resulting in death or serious physical or psychological injury to a patient or patients, not related to the natural course of the patient's illness. 


In raise an incident, the user choose the happend incident into any three of the above outcome type before any data, it will help the quality in further investigation and RCA.
